External Care Coordinator - RN

We are seeking an External Care Coordinator to engage in assessing, coordinating, planning, implementing and evaluating patients in acute care that meet criteria for admissions. Evaluate the medical and nursing health care needs of all possible referrals made to facility.
The ideal candidate will have three years recent LTC experience and a current NYS license BSN preferred.
